• Birth Certificate and/or Passport*
• Immunization Record or Exemption Form*
• Parent/Guardian Photo ID*
• 3 Proofs of Residency*
• Transfer card from previous school (if applicable)
• Last Report Card (if applicable)
• High School Transcript (if applicable)
• 8th Grade Diploma (Grade 9, if applicable)
• Custody Papers (if applicable)
• Student's most recent IEP (if applicable)

* = Denotes that a document is required to enable submission

High School Students ONLY:
All required documents above are required AND must also include previous school's academic documents below:
GRADE 9

• Last Report Card with Final Grades
• 8th Grade Diploma (if issued)
• Most recent NJSLA score (or other comparable state assessment scores)

GRADE 10, 11, 12

• Last Report Card with Final Grades
• Unofficial High School Transcript with Final Grades
• Most recent NJSLA score (or other comparable state assessment scores)
• NJSLA Scores for ELA and Math (Grades 9 & 10)
• NJGPA Scores for ELA and Math (Grade 11)
• Additional Test Scores:
• CollegeBoard Scores - PSAT and SAT (if applicable)
